The Jackson Clinic Senior Care Advantage
The Jackson Clinic wants to help you reach your health and wellness goals. That’s why we developed a special program called The Jackson Clinic Senior Care Advantage. This program is not a new Medicare Advantage plan. Rather, it is a collaborative approach to care delivery that focuses on preventive care and providing care coordination resources for individuals enrolled in participating Medicare Advantage plans. Through this program, the primary care physicians and care teams at The Jackson Clinic are able to offer services that focus on your total health.
Access to Services
The Jackson Clinic patients enrolled in a participating Medicare Advantage plan are able to access the following services through the The Jackson Clinic Senior Care Advantage program:
- Annual Medicare wellness visit
- Cancer screenings and preventive care visits
- Coordinated care with other health professionals and services
- Referrals to specialists and other health professionals
- Patient outreach about upcoming or needed appointments
- Care team support for chronic conditions
Options For Your Needs
Under this program, you have the flexibility to choose one of the participating Medicare Advantage health plans that we and The Jackson Clinic work with. We contract with most major Medicare Advantage plans like Humana, Cigna, Blue Cross Blue Shield of Tennessee and UnitedHealthcare.
If you are already a patient of The Jackson Clinic enrolled in a Medicare Advantage plan with one of the participating insurance companies, you automatically have access to The Jackson Clinic Senior Care Advantage program. Please consult with your The Jackson Clinic physician if you are unsure.
